I see it as a dependence on the trigger, leaning more towards horror. If it's done consensualy to the good guy, then it's usually fantasy. Usually if there aren't other fantasy creatures then it really isn't fantasy, but even then it can be fantasy-like. I guess for pictures it really depends on the mood (Light -> Fantasy, Dark -> Horror).
